The Power of Integration and The Warm Handoff

Here’s a true story from our Clinical Operations Group that illustrates the power of clinical integration, effective communication, and the warm handoff.

A patient came for a routine dental cleaning, but his blood pressure was so high that the dental department couldn’t proceed with the cleaning. Instead of sending the patient home, the dental team contacted the medical department and asked if the patient could see a provider right away.

It turned out that the medical department had lost touch with that patient three months before, and he had stopped taking his blood pressure medication. The medical department was able to get him a walk-in appointment the same day. That warm handoff was a seamless transition from dental to medical care, and the medical team was able focus on his hypertension and blood-pressure management. Once he got that under control, he could come back and get his dental cleaning.
