  • They often have physical, behavioral, and social needs.

  • They often need to rely on multiple delivery systems, including providers for primary care, specialty care, dental care, mental health care, and treatment for substance use disorders.

  • They often have difficulty accessing support services, including healthy food, safe housing, and the ability to get to care providers.

Enhanced Care Management (ECM) is a statewide MediCal benefit that addresses all of these needs by providing intensive coordination of services.

Facktor’s free ECM technical assistance includes help with workflow, documentation, payment reconciliation, and much more.

Facktor Health has been selected as a CalAIM PATH TA Marketplace Vendor.

Facktor would like to talk to you about FREE technical assistance (TA) to help establish and/or strengthen an Enhanced Care Management (ECM) program at your organization, funded by California’s Department of Health Care Services (DHCS).

This funding is part of a $1.85 billion statewide initiative from DHCS to build up the capacity and infrastructure of organizations to successfully participate in the Medi-Cal delivery system and is available on a first-come, first-served basis until it runs out.

Facktor can support you with the following grant-funded projects:

  • Developing strategies to bring the ECM program to profitability

  • Building infrastructure to expand the number of patients served by the ECM program

  • Adding new health plan ECM contracts and/or renegotiating current contracts

  • Optimizing clinical workflows for ECM
