MANAGER
Heidi Chiu is a Manager in Facktor’s Clinical Optimization division and is based in Los Angeles, California. She has spent more than five years working in the FQHC space as both a registered nurse and community health consultant. At Facktor, Heidi has led more than 20 Enhanced Care Management (ECM) projects, with a focus on operational and financial sustainability, improved quality metrics, and optimized administrative workflows. She has also served as an interim ECM Director. Her areas of expertise include integrated care management strategies, Medicare clinical operations, EHR workflows, and 340B programs.

Before joining Facktor, Heidi worked as a Registered Nurse at an FQHC in Washington, DC, where she led COVID-19 testing, immunization, and needle exchange clinics, while also triaging walk-in patients and phone calls. She earned her BSN from the University of Pennsylvania and her Master of Public Health from UCLA. Heidi is especially passionate about community health and the mission of FQHCs. Outside of work, she enjoys exploring cafés and grocery stores across Los Angeles, spending time with her cat Miko, and reading.
