SENIOR ASSOCIATE
Based in the Greater Chicago area, Dylan Meehan is a Senior Associate in Facktor’s Clinical Optimization division, where he supports healthcare organizations in advancing care management strategy and operational performance, with a focus on Enhanced Care Management (ECM) and CalAIM initiatives. He brings a hybrid background across care model design and operational optimization, with experience supporting CalAIM contracting efforts and broader care management program development, including workflow design, performance management, financial forecasting, staffing model optimization, implementation support, and strategic sourcing of population health and case management platforms.

His work is primarily focused on FQHCs and provider organizations, where he helps strengthen care management operations by developing KPIs and performance benchmarks, refining workflows, and supporting staff training and operational alignment. He has also contributed to ECM leadership engagements, supporting interim care management functions and program execution. In addition, he has supported Operational Site Visits (OSVs), New Access Point (NAP) applications, and care management recruitment for both frontline and leadership staff.
